What to expect at your consultation

Once you have completed this form, a member of our team will reach out and book your consultation with you.

You will visit Herzig Eye Institute and be welcomed by our patient service representatives.

Our ophthalmic technicians will thoroughly conduct your diagnostic measurements that will be reviewed with you by our doctors.

Once the doctor has recommended a personalized procedure, you will meet with a refractive consultant to discuss surgery plans.

At Herzig Eye Institute, we are pleased to offer complimentary consultations for our High Definition Vision™ solutions such as ZEISS ReLEx SMILE, LASIK, PRK, Intraocular Collamer Lens (ICL), or Refractive Lens Exchange (RLE).

Cataract consultation are covered under OHIP.
